In addition to the around 40 young people we see every day out our centre, New Horizon provides the only youth-specific street outreach service to young people facing homelessness in London. We use our extensive housing expertise to place young people who are currently or at risk of rough sleeping into accommodation that is appropriate and safe, whilst encouraging them to access the day centre to benefit from the wide range of services we offer. Herts Mind Network (HMN) has a partnership with Hertfordshire Partnership Foundation Trust (HPFT) to deliver a one-to-one outreach service for people who are in hospitals across Hertfordshire.
There is an emergency department arm of the project and will be based at Watford General Hospital working in A&E departments, with the Mental Health Liaison Team, and hospital wards facilitating hospital discharge for people with mental health need.

The Bounce Back Worker will be based at Watford General Hospital and HMN Centres, promoting joint working between Watford A&E, creating a seamless pathway into community support for clients recovering from mental ill health.
The aims of the Bounce Back service are to provide up to 16 sessions of emotional and practical support and advice & information to patients. Rough sleeping pathways only work when the system around people moves with urgency, consistency and accountability. This role is about making sure people sleeping rough, or at risk of returning to the streets, are not left waiting, drifting between services, or losing accommodation because barriers have not been gripped quickly enough.
As Outreach Housing Pathway Coordinator, you will work within Camden’s integrated Rough Sleeping and Outreach Hub to help people access and sustain appropriate accommodation. You will hold oversight of where people are in their housing journey, identify what is blocking progress, and work with outreach teams, Navigators, hostels, pathway providers, community safety partners and statutory services to keep plans moving.
You will focus on the points where pathways can break down: delays, exclusions, unsuitable offers, unresolved risks, street activity linked to accommodation, or support needs that have not yet been fully understood or responded to. Through assertive engagement, practical case coordination and strong partnership working, you will help people stabilise their accommodation and reduce the risk of returning to rough sleeping.

A bit about us:
WellChild, based in Cheltenham, is the national charity for seriously ill children and their families. Through a range of practical and emotional support, training and information programmes, WellChild gives children and young people with complex medical needs the best chance to thrive at home.
Purpose of the Role:
The garden makeover (Helping Hands) team provides practical support to families by enlisting the help of corporate volunteer teams from companies across the UK to undertake one-off improvement projects to create safe, accessible sensory gardens for children and young people with complex health needs.
There are six key parts to this role:
1) Meeting with our beneficiary families to identify the needs for their outside space. Using empathy and active listening to ensure you can gather all the relevant information for a successful transformation.
2) Project design and procurement of materials to deliver a suitable space, that not only meets the beneficiaries needs, but is also delivered within our time and budget specifications.
3) Compile all relevant project documentation and engage with the allocated team of corporate volunteers.
4) Deliver your design over two days (which may include overnight stays depending on project location) with an enthusiastic but often unskilled group of corporate volunteers. You will need to inspire and empower these teams in all elements of the delivery of your project and be the subject matter expert as the sole representative on site from WellChild.
5) Complete post makeover administration on our equipment and communication with the corporate volunteers.
6) Management of each makeover’s financial budgets, for equipment, materials, travel, accommodation as well as a joint monthly tool and equipment budget.
Fairly regularly to complete these garden makeovers, there is a degree of working away from home. For example, you might work on a five-week rotation consisting of two weeks conducting one makeover, over two days, each week. This is predominantly over the Spring and Summer seasons. Speaking for Ourselves is an exciting new programme funded by the National Lottery Heritage Fund that will collect and share the history of the care-experienced campaigning movement since the 1970s. It was conceived within the care-experienced community, emerging from long-standing discussions about how poorly the history of the care-experienced campaigning movement has been recorded—and how easily its achievements risk being forgotten.
This important programme puts the voice of people with lived experience of care at the centre. The Speaking for Ourselves programme delivered by Coram is guided by a majority care-experienced Programme Board and aims to work with the wider care experienced community. The Programme Board has an advisory and representative role, to guide the successful delivery of the programme and make key decisions that shape its impact.

About the role
The Care Experienced Engagement Coordinator role presents a unique opportunity to work directly with the care experienced community to bring to life this untold history. The role will hold primary responsibility for the engagement with the community, and care experienced individuals and allies including outreach, liaison and ongoing relationship management. Along with the team they will need to ensure the programme is delivered sensitively and embed a trauma informed approach.
We are looking for a person who has direct experience of successfully working with diverse groups of people with lived experience of the care system – personally or professionally, along with experience of working with young people and adults to develop and deliver projects preferably in a heritage / creative context.
The post holder will report to the Programme Manager, and will be responsible for working with them, the team, the Programme Board and Coram to deliver the programme.
