Overview

An exciting opportunity has arisen for a Non-Medical Prescriber to join Change Grow Live Tower Hamlets Service. You will be an integral member of a multi-disciplinary and multi-agency team delivering recovery orientated alcohol and drug intervention services.

Our NMP's are integral members of our multi-disciplinary and multi-agency teams, delivering recovery orientated alcohol and drug intervention services. Offering care for presenting clients, prescribing and/or making amendments to prescriptions of opiate and/or alcohol substitute treatments and other related medications, they take a lead role in developing the prescribing clinic, acting as a point of contact for other workers and partner agencies around prescribing issues.

Hours: Full Time, 37.5 per week

Salary: £47,792.23 - £53,329.99 Dependent on experience (Based on full time hours, pro rata for part time)

Contract: Fixed Term until 1st October 2027 (with the potential to extend)

Responsibilities

About the role: 

• Assess, diagnose, plan; implement and evaluate treatment/interventions and care for service users. 
• Prescribe and review medication for therapeutic effectiveness appropriate to service user needs and in accordance with evidence-based practice and national and local protocols, and within scope of practice. 
• Prioritise health problems and intervene appropriately to assist the service user in complex, urgent or emergency situations seeking immediate advice and direction from the Clinical Lead if needed. 
• Assess, update, and refer service users for BBV vaccination and immunisation programmes. Support service users to adopt health promotion strategies that promote healthy lifestyles. 
• Communicate effectively with service users and carers, anticipating barriers to communication and recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating. 
• Maintain effective communication within the organisational environment and with external stakeholders, acting as an advocate for service users and colleagues where necessary. 
• Produce accurate, contemporaneous, and complete records of consultation, consistent with legislation, policies, and procedures. 
• Assess effectiveness of care delivery through self and peer review, benchmarking, and formal evaluation. Be actively involved in the annual appraisal system. 

About you: 

• Qualified pharmacist with current GPhC registration and two year’s post qualification experience OR First level registered nurse with a current PIN AND three years post qualification experience 
• Qualified Non-Medical Prescriber 
• At least one year’s relevant and recent experience of working within substance misuse treatment. 
• Sound and demonstrable knowledge of harm reduction and health promotion interventions 
• Ability to assess and recovery plan service users and formulate written reports, as necessary. 
• Ability to communicate confidently and effectively, verbally and in writing. 
• Full driving licence and access to own transport, or ability to demonstrate easy movement between services, if necessary

About the role:

The MARAC Administrator will support the MARAC Coordinator in the administering the MARAC process. The post holder will be co-located in Brent and will be required to attend MARAC meetings in various locations as required. There will also be a requirement to attend Advance’s Head office.

The MARAC Administrator will process MARAC referrals and flag to the MARAC Coordinator any referrals that have insufficient information or may not meet the MARAC threshold. The post holder will also compile the MARAC At Risk List for approval by the MARAC Coordinator, including case details of survivors and perpetrators who will be discussed at the MARAC.

About You:

To be successful as the MARAC Administrator you will need the below experience and skills:

• An understanding of violence against women and girls with a particular focus on the dynamics of domestic violence (physical, emotional and sexual violence, so-called ‘honour-based violence’, forced marriage, stalking and harassment) and its impact on women, children, families and communities.

• Experience of office management and administration roles and experience of maintaining data systems and producing regular monitoring reports

• Accurate and methodical approach to dealing with data processing, filing systems, facts and figures

• Excellent literary and administrative skills including ability in information technology, data collection and in-depth working knowledge of Microsoft Excel and Word

• Good interpersonal and communication skills

• The ability to be flexible, working as a team or independently as required to support the service and colleagues

• An ability to work in partnership with relevant statutory and voluntary groups, to demonstrate the ability to develop and maintain strong constructive working relationships

• An ability to cope in stressful situations; able to multi-task and prioritise, remaining calm under pressure

We are looking for a Community Support Worker to join our team. The Primary Care Network (PCN) Service works in partnership with GP surgeries in Hertfordshire to provide advice, information, onward referral and holistic support to individuals who are experiencing mental ill health or need support with their mental wellbeing.

Community Support (PCN) Service Objectives

• To improve the mental wellbeing of people experiencing mental ill health.
• To increase early access to help for people experiencing mental ill health, by providing a clear and effective pathway to services provided by Herts Mind Network and other third sector and statutory providers.
• To contribute to an improvement in individual mental wellbeing.
• To remain a source of independent support for all clients.
• To reduce the use of crisis services by people experiencing mental ill health without positive outcomes for the individual.
• To contribute to an improvement in the proportion of people with mental ill health who are accessing support.
• To provide an assertive outreach approach for clients who are in crisis and/ or those that may have complex or multiple needs.

The Successful Candidate will

• Provide advice, information, onward referral and holistic support to clients who are experiencing mental ill health or need support with their mental wellbeing.

Job Title: Housing Support Worker

Location: Brent. Step‑free access is available at our main office. However, due to the nature of the role, you may be required to visit homes and other buildings that do not provide step‑free access

We're hiring a Housing Support Worker to join our floating support service which is based in Brent. The service supports adults who have been or are at risk of homelessness, have experienced social exclusion, have previous criminal convictions, suffer with their mental health, and substance misuse, and/or could have a learning disability. You will support participants on their journey towards recovery, reablement, and independence, all within a compassionate, trauma informed environment.

You will also work closely in partnership with various organisations to support the different needs of our participants, which may include signposting. A stimulating and rewarding opportunity has arisen for a Practitioner Psychologist with a Forensic background. You will work alongside the Group Psychological Lead and other members of the team within our Independent Approved Premises (IAP) to deliver psychological services to residents and staff. In this role, you will apply your skills and knowledge in building new teams and expand our operational delivery within our IAP. You will play a strategic role in our organisational growth with new services, with a focus on shaping service delivery; offering specialist advice, supervising assistant psychologists and postgraduate research.

Our IAP's support people who have left prison and are based within a residential setting to reintegrate into the community. They are given support and guidance to aid their reablement, resettlement and rehabilitation post prison. You will contribute towards them achieving goals necessary for their personal development and move on within the community. 

Key Responsibilities include:

• Providing high quality psychological services, contributing to the delivery of Approved Premises aims as specified by the Ministry of Justice.
• Ensuring all services are delivered in accordance with HCPC standards of conduct, performance and ethics.
• Demonstrating clinical leadership by setting direction for the psychological service and driving high standards for quality, safety and ongoing improvement.
• Exercising clinical responsibility for residents’ psychological care, showing skill and judgement in selecting appropriate assessments and interventions.
• Consulting with other parties contributing to a resident’s diagnosis, formulation, treatment and risk management in the community.
• Create opportunities to collaborate with peers including AP-based psychologists nationally and OPD pathway psychologists delivering services to APs.
