Pathways is a specialised welfare advice service that provides information, advice, advocacy and representation for Disabled adults (over the age of 18) living in the Richmond Borough. We work with clients, supporting them to navigate the complex Housing, Benefits and Social Care systems. In this role, you would be providing information, guidance and advice to Disabled people and their families, enabling them to have more choice and control over their lives.
We are also part of a larger network of Disabled-led advice services. Working together, we have developed pathways for Disabled trainee case workers, collated data on Housing and Benefits issues and have fed into important consultations and campaigns.

About the role
An exciting opportunity has arisen to join the Social Work Service at St Joseph’s Hospice, based in Hackney. As a Senior Social Worker, you will work as part of a committed and creative team, which offers innovative social care interventions and support to people with palliative and end-of-life care needs, and to their families, carers and children.
The social work team works in collaboration with other clinical professionals and volunteers across all areas of the service: inpatient, outpatient and community. The team includes social workers, a dedicated carers service lead and welfare benefits lead. In addition to clinical case work and line management responsibilities, the Senior Social Worker deputises for the Social Work Manager in their absence, providing cover for day-to-day operational matters.

About KLS’ Future Foundations education team
Future Foundations is an education programme of Katherine Low Settlement. Since 2004, KLS’ Future Foundations education team of 10 part-time staff and over 100 volunteers, have supported young refugees and their families in Battersea and the London Borough of Wandsworth to thrive in their education. Through mentoring, family support, casework and homework clubs, we provide the tailored support each young person and their family (if they have one) needs to overcome the barriers to education they face at home and school.

Purpose of This Role:
This is a strategic role, funded by the Sir Halley Stewart Trust, to shape future policy and practice in how faith communities support social prescribing for the benefit of local communities. This includes exploring the role of faith as a strategic partner in the government's neighbourhood health agenda. Building on the work of the current postholder, and previous work by NASP and organisations like Theos and the Good Faith Partnership, this role will take the lead at a national level by influencing, shaping and convening partners to unlock and unleash the significant resources of faith groups in contributing towards holistic healthcare delivered within the community.
The purpose of this role is to lead and co-ordinate NASP’s work on social prescribing with partners across the faith sector and enable a better understanding of how to work effectively with faith communities through social prescribing, and the role that faith and belief plays in supporting good health and wellbeing. The role will work to improve accessibility of community support through social prescribing by exploring the barriers and opportunities in faith communities and the health sector. The role will have a particular emphasis on health inequalities and explore opportunities for faith groups’ reach into deprived communities and ethnic minority communities, recognising that faith communities may be most trusted precisely where health inequalities are most acute.
The role sits in the National Leads & Evidence team, led by the Executive Director of Strategy and Partnerships. The postholder will work alongside NASP colleagues who lead on Healthcare integration; Evidence and Insights; International Social Prescribing; and connections with sectors that provide community activities and support such as the natural environment, physical activity, historic environment and arts and culture.

Role Overview:
• Act as the faith lead within NASP, being the point of contact and key advocate for faith communities’ involvement in social prescribing, across all major traditions.
• Represent and develop faith groups’ engagement in NASP’s existing activities, programmes and events including workstreams in NASP to build the capacity of Social Prescribing Link workers (SPLWs); support the community assets that SPLW’s harness in their work; and connecting across different Government Departments to explore how social prescribing connects with strategies related to employment, youth, education and community cohesion.

Purpose of the post:
The Senior IAPT Practitioner will play a key role in delivering and developing Mind in the City, Hackney and Waltham Forest’s specialist Talking Therapies provision for African Caribbean Heritage (ACH), LGBTQIA+ and Global Majority communities.

When young people in care turn 18, all the support they’ve relied on can fall away, literally overnight. Suddenly, they have to navigate adulthood on their own. This would be tough for any teenager, even if they could fall back on their family for help. So it’s not surprising that two out of three care-experienced young people feel unprepared to leave care. Too many still don’t get the support they need to make it safely past this cliff edge.
Care leavers aged 18-21 are nine times more likely to experience homelessness than their peers.
Settle exists to change this. The Role
We are currently looking for an enthusiastic, highly motivated, and experienced support worker to join our Young People Support Team based in Hackney. You will provide client directed, person centred support to help young people achieve personal goals and aspirations for independent living, training, employment, and education. IPS – Employment support
For people who need mental health support, getting back into work is a vital step on their recovery journey. The evidence backs this up, but although 90% of people with severe mental illness want to work, only 8% of them are in paid employment.
If you would like to join our team and champion our mission and help people who need employment support to get back into work. You will be supporting clients who are under the care of Secondary Mental Health services to gain and retain paid employment and will be based within a clinical team.
This is an incredibly rewarding role. You'll have the opportunity to transform the lives of clients, to give them hope, direction and support with their recovery journey alongside other key professionals. This is also a challenging role, so you’ll need to be empathetic, adaptable and dedicated to finding clients a role that’s right for them.
As an Employment Specialist, you’ll build a good rapport with your clients, gaining a real understanding of their key skills, their aspirations and their career goals and finding them opportunities to match. You'll also spend time building productive relationships with employers in order to identify and negotiate job opportunities in the hidden labour market.
Successful applicants for this post will receive training in the IPS approach, giving you the tools, you need to provide expert support and advice to clients, staff within your clinical team (e.g. Psychiatrist, Social Workers or Mental Health Nurse), while also building positive relationships with employers, opening doors and changing perceptions around mental health.

Job Summary
To provide support into employment for people with mental health problems including:
· Managing a caseload of clients who wish to return to paid work, enabling them to find or retain employment by liaising with work and training providers, accessing benefits advice and by personally providing on-going support to clients in job search and after gaining employment, in accordance with evidence based supported employment practice such as the Individual Placement and Support (IPS) model of supported employment.
· Link with local employers and voluntary bodies in order to secure employment opportunities within the community

Key responsibilities

Main duties
· Manage a caseload of people who have experienced mental health problems who wish to work.
· Prepare individuals for employment by assessing each person’s individual vocational needs which might typically include identifying strengths, help with benefits, support networks, travel to work plans, etc.
· Co create employment action plans with the individual, and provide this to the care co-ordinator and other professionals where appropriate.
· Assist individuals in job search and prepare them to apply for employment/education, liaising with colleagues as required e.g. Disability Employment Advisors, Jobcentre Plus, DWP contract providers and local colleges.
· Contact and meet with employers to identify job opportunities for individual clients.
· Have a proactive approach in developing jobs tailored to suit individual clients’ needs by contacting local employers, discussing mutual requirements and negotiating opportunities.
· Accompany job seekers to interviews if required.
· Provide education and support to employers, as agreed with the individual and provide job retention interventions to people at risk of losing their employment, negotiating adjustments to the job role with employers to enable the person to retain their employment.
· Support clients in the workplace by supporting them to learn the job role if necessary, and/or negotiating with employers to adapt the role at the start of employment or if difficulties arise in the workplace.
· Identify any barriers for return to work and develop appropriate strategies to overcome those barriers for clients who wish to gain or retain open employment maintaining a positive attitude to addressing challenges.
· Contribute to the opportunities for an individual’s increased social inclusion by developing partnership working with voluntary sector agencies, and linking with primary care employment and job retention initiatives.
· Promote the Mindful Employer initiative and best practice in mental health and employment within the mental health services and in other organisations in the local area.
· Maintain and develop close links with referrers, care co-ordinators and Community

Oasis Youth Support at North Middlesex Hospital is seeking a dedicated Outreach Worker: Violence Against Women and Girls Specialist to accompany young women who attend North Middlesex Hospital A&E department due to a violent or aggressive incident, on their journey to make positive choices with the aim of breaking the cycle of violence through 1:1 mentoring.
You will be part of a large team based at the hospital and supported by the Youth Support Project Coordinator, as well as the wider Community Hub Team based in Ponders End, Enfield.
The key responsibilities within the role are:
· Support young people and develop a bespoke mentoring programme that meets their needs
· Develop and evaluate a model of care that enables young people to benefit from community based interventions

About the Service and the Role
